What is Palliative Care?
Shore Memorial Hospital’s Palliative CARE Program
offers patients with serious illnesses and their families Comfort
with Autonomy, Respect
and Empathy. It is comfort care that provides
patients relief from pain, symptoms and the stress of illness, whatever
the diagnosis.
The goal of palliative care is to help individuals live comfortably
and to provide them with the highest quality of life, while addressing physical
comfort, emotional health and meeting the spiritual needs of patients and
their families.
What Conditions Are Addressed?
Working closely with your physician, Shore Memorial’s Palliative
CARE Team supports the whole patient and their loved ones who are
sharing the burden of serious illness. The CARE Team provides:

Shore Memorial's Care Team
Palliative CARE Team helps you better understand your condition and your
choices for medical care. The team includes your attending physician, nurse,
social worker, chaplain, pharmacist, dietician and other specially trained
members, along with the CARE physician and advanced practice nurse.
