The Social Service Department at Shore Memorial Hospital works to address the emotional, medical and physical needs of the patient and their family before, during and after admission.

During your stay in the hospital the social worker assigned to your floor is available to answer your questions, help you understand your illness, coordinate your plan after discharge and support you through your stay.

Many of the services provided include:

• Emotional support
• Identification of your needs at discharge
• Assistance with various community programs
• Support through drug and alcohol treatment
• Addressing psychiatric needs and coordinating services
• Discharge planning
• Placement in rehabilitation and nursing facilities
• Locating resources to address financial distress due to medical condition
