Shore Memorial Hospital’s Palliative CARE Program offers patients with serious illnesses and their families Comfort with Autonomy, Respect and Empathy. It is care specifically designed to provide patients relief from pain, shortness of breath and anxiety, and the stress of illness, regardless of diagnosis.
The goal of palliative care is to help individuals live comfortably and to provide them with the highest quality of life, while addressing physical comfort, emotional health and meeting the spiritual needs of patients and their families.
Working closely with your physician, the CARE team provides rehabiliative, emotional and spirtual assistance and management for difficult symptoms such as:
Additonally, the team can help patients with spritual needs such as prayer, religious rituals and sacraments, as well as assist with discharge needs, home care, rehabilitiation, long-term care and medical equipment needs after discharge.
The Palliative CARE team helps you better understand your condition and your choices for medical care. The team includes your attending physician, nurse, social worker, chaplain, pharmacist, dietitian and other specially trained members, along with the CARE physician.
