Chronic Kidney Disease

If you have high blood pressure or diabetes, ask your doctor about your GFR. The GFR is your Glomerular Filtration Rate, and it tells you and your doctor how well your kidneys are functioning. If it's below 60, it means your kidneys aren't working as they should.

But there is help. Shore Memorial's Chronic Kidney Disease Management program is the only service of its kind in the region. It can help you control and maintain optimal kidney function.
